This program will review the best practice roles and functions of the social worker and the RN case manager in order to differentiate the similarities and differences between the two roles. Included will be a review of the standards of practice for case management as well as the unique elements of the social worker’s role in psychosocial assessment, planning, and intervention both in the hospital as well as in the community.Referral criteria for referral of a patient to the social worker (high-risk screen) will be discussed. Strategies for integration of the roles of the social worker and the RN case manager will provide a framework for ensuring that the two positions are different yet complementary. The role of the social worker in community-based case management programs will also be reviewed as well as how to identify the best ways to optimize the roles in the community where there are large numbers of patients that may require case management services. However, the roles have sometimes become unclear as to which professional performs which elements of the case management roles and functions. This webinar will review the differences and similarities between the roles of the social worker and the nurse case manager in both acute care and community-based settings.
