This face-to-face learning will look at how a team-based approach is used to carry out the patients required care. You will analyse the benefits of a PCMH and gain a better understanding of the integration of care and how to increased professional satisfaction. During this session you will explore how fulfilling the goal of helping patients while making clinical work easier and delivering a more efficient services.
